Who we are

This website is operated by ZahavMD PLLC, a medical practice in Chicago, Illinois ("we" or "us"). You can reach us about this notice at [email protected].

No physician-patient relationship

Visiting this website, joining the membership waitlist, or contacting us by text, email, or phone does not create a physician-patient relationship. That relationship begins only when we agree to provide you with care and you have completed our intake and consent process. Nothing on this website is medical advice.

Please do not send health information

Do not enter or send sensitive personal or health-related details through this website, by text message, or by email. Text messages and email are not a secure way to send health information. Medical information is gathered only during care, through appropriate channels.

What we collect

If you join the membership waitlist, we collect the details you enter: first and last name, phone number, email address, ZIP code, and whether the membership is for an individual or a family. We also record the date and time you submitted this form.

If you text or email us to ask about a visit, we receive your phone number or email address and whatever you choose to write.

How we use it

We use this information only to contact you about your interest in receiving medical concierge care, such as scheduling questions, answering your inquiry, and letting you know when membership becomes available. By joining the waitlist, you agree that we may contact you by phone, text message, or email for these purposes. Message and data rates may apply, and you can opt out at any time by telling us. We do not sell your information and we do not share it for advertising.

Who else handles your information

We use outside service providers to run the website and to communicate with you: Cloudflare for website hosting and the waitlist database, and Google for email, text messages, and calls (Google Workspace and Google Voice). These providers handle information on our behalf under their own terms and privacy policies. Like most websites, our host may keep standard technical logs, such as IP address and browser type, for security and reliability.

Security

Waitlist details are stored in a database that is protected by access controls and accessible only to authorized ZahavMD staff. We use reasonable safeguards, but no method of transmission or storage is completely secure, so we cannot guarantee absolute security.

How long we keep it

We keep waitlist information until you ask us to remove it, until we close the waitlist, or until you become a patient. If you become a patient, the information you gave us may become part of your records. We may keep limited records where the law requires us to.

Cookies and tracking

This website does not use advertising cookies or analytics trackers.

Removing your information

You can ask us to remove you from the waitlist or delete the details you gave us by emailing [email protected]. We will respond within 30 days, except where we are required by law to keep certain records.

Children

This website is intended for adults 18 and older. We do not knowingly collect information about children through this website. Family memberships are requested by an adult.

Patient records

This notice covers this website only. Information we create or receive while providing medical care is handled under separate rules for medical records and will be explained to patients directly.

Patient records, intake forms, secure messages, and billing are kept in Healthie, a secure electronic health record service we use for our patients. The “Patient login” link on this website takes you to Healthie’s sign-in page; this website does not receive or store what you enter there.

Changes

We may update this notice from time to time. The date at the top shows when it was last changed.
